Description

This is a specialised service for adults with a learning disability, jointly provided by Aberdeenshire Council and NHS Grampian. The service also gives support to families and carers. The service is provided in day centres, health centres and the client's own home.

Aims

To provide a health and social work service that meets the care needs of adults with a learning disability.

Services/Activities

Community Learning Disability Team members consist of:

  • Consultant Psychiatrist
  • Care Managers / Social Workers
  • Community LD Nurses
  • Speech and Language Therapist
  • Clinical Psychologist
  • Physiotherapist
  • Occupational Therapist
Referral:

The Central CLDT operates an open referral system where referrals will be accepted from individuals, carers, next of kin, Day Centre Officers, Care Managers and GPs.
