Description

The Moray Community Learning Disability Team offers support to people with a learning disability, their parents and carers, and to voluntary and professional groups dealing with people who have a learning disability. The Team is run jointly by NHS Grampian and the Social Work Division of The Moray Council.

Services/Activities

The Team consists of three community nurses (RNMH), a clinical psychologist, care managers, a community support manager, two psychology assistants, a physiotherapist, a GP clinical assistant, a speech and language therapist and a consultant psychiatrist.

The Team:

  • carries out assessment of need and sets up packages of care, health issues
  • provides information, training, education and counselling support for people with a learning disability, their carers and for other professionals groups
  • can put clients in contact with specialised services

There is also an Epilepsy Clinic with referral by a GP and an epilepsy course available for carers.

Referral:

Usually by GP but members of the public can phone the team for advice. Referral via a Single Shared Assessment (SSA)

 

Cost:

Assessments are free, so is direct staff intervention / support via CLDT staff members. Community support, care packages may require a Finance Assessment and client contribution.

Highfield House, Elgin

Further access information:

Office premises are on the first floor are inaccessible to wheelchairs.

Most visits are made to the client or carer in their home or workbase or clients are seen in the consulting rooms at Highfield House.
