Registering as an Information Provider (IP)

 

 

Why do I need to register?

You cannot enter records about your service unless you have first registered as an Information Provider (IP). As part of quality control, Grampian CareData will check that IPs are genuine and have the authority to supply information on behalf of their organisation. If there is a question about meeting the Inclusion Criteria, you may be contacted to discuss your service.

Once your registration is approved, an IP Account is created for you wth your contact details. These are used to send you a reminder when the annual review of your records is due or to contact you to clarify any information supplied.

Important note: Your IP account details are not published and are held for administrative purposes only. The records attached to the IP account are published. Please ensure your IP account is updated as well as your records!

How to Register

Go to the Information Providers login page - there is a link on the Home Page and at the top right of every page. Once there, please read the Data Protection statement and the Inclusion Criteria. Then check the box to say you have read them and click on Register.

Complete the form that appears and again click on Register. Helpful hints and tips are provided throughout the record entry form and instructions are below.

You will be sent an email once your account has been approved. You will not be able to log in until the account has been approved. Once the account is approved, you will be able to add records to it. See Adding and Updating Records to get started.

Filling in your IP Registration Form

Required Fields are marked with a *

*Organisation name

The name that your organisation is usually known by, in full

*User Name

This is the name you will use to log in

*Password

Your password, that was chosen at registration, should appear here

*Email address

Use the contact email address of the person responsible for providing and maintaining your information or an email address that they can access

Contact name

This is the name of the person responsible for providing and maintaining information on behalf of your organisation or group

Salutation

Enter the title you wish to use, eg Mr, Mrs, Dr. You may leave this blank if you wish

Position

The job title or position of the person supplying information, if relevant

Address, Town/City, Regional Area and Postcode

Although the postal address will not normally be used to contact you, it would be helpful to us for administrative purposes if these fields can be comleted. If you would prefer never to be contacted by post, please leave these fields blank

Telephone number

Telephone contact details can often help us clear up a query quickly, although contact will usually be via email. However, if you would prefer not to be contacted by telephone, please enter 0
